MCAP for Rehabilitation - Alder Hey Children’s NHS Foundation Trust
ISSUE
MCAP data identified patients that are non-qualified on admission to the ward following orthopedic or neurosurgery for acute rehabilitation. MCAP has demonstrated that these patients do not require acute rehabilitation, and would be appropriately managed in the community. However, there is no local community facility available and meant patients waited a significant length of time in hospital for their rehab placement.
SOLUTION
The Models of Care project explored the opportunity for step down rehab provision using MCAP data demand analysis and the Trust commissioned medical beds to be re-designed to include rehabilitation step-down beds within the Trust.
OUTCOME
Four surgical beds have been released and the rehab patients are now in the right bed/level of care. This has reduced the number of patients delayed due to awaiting rehabilitation by 37%.
