Using MCAP to Identify delays in Medication / Pharmacy - The Walton Centre NHS Foundation Trust

ISSUE


 

MCAP data highlighted that delays relating to TTOs were due to internal delays. The Trust do not have a pharmacist on-site and have to collect locally at 2 pm and 5 pm. A deeper dive of the data revealed:

1.       No cover at weekends for dispensing of TTOs causing delays in the discharge of patients.

2.       The timings of ward rounds often delayed orders and pharmacy collections.

SOLUTION


 

1.  A list of prescribers within the Trust is shared with all wards.

2.  An additional collection round added at 12 pm.

3.  Team explored pre-packed drugs options.

OUTCOME


 

TTOs is no longer a top 5 internal delays for the trust. The over all figures for internal delay days reduced 48%.
