Using MCAP to Reduce Overnight Stays - Alder Hey Children’s NHS Foundation Trust

ISSUE


 

MCAP data identified the specific improvement patient flow processes that would support patients through their pathway efficiently: minimising delays and reducing time spent in hospital. Enabling children and families to spend less time in hospital when it is safe and appropriate to do so.

SOLUTION


 

1.       Implementing SAFER model using MCAP as the main enabler

2.       Expanding day surgery service

3.       Pre-operative assessment and planning

a.       Step 1 - Introduction of Hospital Manager of the Week

b.       Step 2 - 13:00 bed meeting moved to 12:0o

c.       Step 3 - Introduction of 15:30 forward look meeting with Crib Sheet

d.       Step 4 - Introduction of Crib Sheet at 8:30 and 12:00 bed meetings

e.       Step 5 - Audit impact of Patient Flow in Control

OUTCOME


 

In the targeted wards:

1.       A decrease in the number of delayed discharges by an average of 66% in 6 months.

2.       A reduction in the average LoS by an average of 28% in 6 months.
